A confined space permit is required for any operation involving entering a space that has limited or restricted means of entry or exit or is not designed for continuous occupancy, and contains one or more of the following: the ability to engulf or asphyxiate the entrant, a potentially hazardous atmosphere, or any other recognized serious safety or health risk.

This permit is to be completed in itis entirety by the responsible party (the person supervising the work) and provided to the Officer of Facilities Development for approval prior to confined space entry.
